Important Health Coverage Tax Documents

1094s/1095s are being issued to active employees per Affordable Care Act requirements. Non-employees (e.g. retirees) who are enrolled in a City-provided health insurance plan may request a copy of Form 1095-C by e-mail to hrdirector@cityofwilliamsport.org, by calling 570-327-7571, or by U.S. mail to Human Resources, 100 West Third Street, Williamsport, PA 17701.
